what does this do, I have about 500k hands. is it worth doing?

It's not necessary to rebuild the database cache unless a member of the support team has recommended it but you should rebuild the custom cache if you ever add new custom statistics:

Regular manual housekeeping isn't necessary with PokerTracker 4 although a reindex can help with some performance issues but if you are having an issue I recommend reporting that to us in a
Support Ticket with your log file so we can advise accordingly:
