The bet lines are grouping data from the perspective of the player making the bet. The data for players who did not make a flop bet is on the 'No Bet' line and the populations 'Call F CBet' is 45.21%. If you want a report that shows the fold frequency for each sizing then that would require a custom statistic that groups the report by the size of the bet
faced (
cash_hand_player_statistics.val_f_bet_facing_pct) instead of the size of the bet
made (
cash_hand_player_statistics.val_f_bet_made_pct). See
this guide for the basics on custom statistics creation and
this guide for a deeper walkthrough. The latter was written for PokerTracker 3 but the techniques all apply to PokerTracker 4, the interface is just slightly different.
You can use the existing statistic as a template for creating a new statistic but bear in mind that for facing bets the percentage of the pot faced is how much a player has to call - it's the pot odds and the bet amount is included. For example if 'Player A' makes a flop bet of 50% (5/10 * 100) then 'Player B' is facing a bet of 33% (5/15 * 100). This definition is necessary in order to distinguish between situations in multiway pots where a bet has been called before a player or not as that changes the pot odds.

The stat groups by flop bet size so naturally when that is true the flop CBet frequency for those hands where the flop bet was a cbet will be 100%. Also please note that the hands on each line will not all be cbets and will include flop floats and flop bets in limped pots (you can mouseover a statistic to display the times/opportunities for a statistic).

You can see the frequencies of turn cbets in that report based on the flop sizing. For example when a player cbets flop < 1/4 pot they are following up with a turn cbet 65% of the time.

Yes. That line includes all data for players who did not make a flop bet including those who folded preflop.
