Sad to hear the issue is back. But lets try to get it fixed then!
If the steps that worked the first time did not make it, then we need to dig further into this.
Please open a support ticket and add to it the log files after the issue happens and a sample of about 10k hands from the pokerroom we have isolated for us to try and replicate this on our end.

Just to try and isolate this.
The issue happens after or during playing live at the ipoker network mostly.
It manifests by closing on its own Notecaddy and giving the error warning.
IF I launch PT4 and exit right after, no error shows up?
